Join the Dynamic Support Register to reduce hospital admission risk

Join the Dynamic Support Register if you have a learning disability and/or are autistic, at any age, and are at high risk of a hospital admission or losing your placement. It helps get you the right support early.

People on the register are placed in priority groups by risk level: green, amber, red or blue. If you are in the amber or red group, you get a keyworker from Barnardo's, giving emotional, practical and advocacy support.

A professional can refer you (with your permission), or someone with parental responsibility can refer you, or you can refer yourself.

For anyone with a learning disability and/or autism, of any age, most at risk of mental health or specialist hospital admission or losing their placement. Identifies the right support early, working alongside CETRs.

Referrals can come from professionals (with consent), someone with parental responsibility, in the person's best interests if they lack capacity - or people can self-refer.
